Privacy Policy
How Hale Street Insurance collects, uses and protects information submitted through this website.
Effective date: November 1, 2026
This Privacy Policy explains how Hale Street Insurance, LLC ("Hale Street Insurance," "we," "us," or "our") collects, uses, shares, and protects information when you visit www.halestreetinsurance.com (the "Site") or send us information through it.
We wrote this in plain English. If something is unclear, contact us at [email protected].
Who we are
Hale Street Insurance, LLC is an independent, church-focused insurance brokerage based in Boxford, Massachusetts. We serve churches nationwide, including multi-campus churches and church schools and childcare programs. We also serve faith-based nonprofits and businesses.
We are licensed as an insurance agency in Massachusetts (license #3002417405). We place some coverage through our partner agency, SmartChoice, so we can serve organizations in every state. Details are on our Licensing page.
This policy covers information you give us through the Site, through online intake forms we send you, and when you call, text, or email us. Once we place insurance for you, information about that insurance may also be covered by separate notices, described in "Insurance privacy notices" below.
Information we collect
Information you give us
We collect what you choose to submit through forms on the Site. Depending on the form, this may include:
Contact details, such as your name, role, work email address, and phone number.
Church or organization details, such as your church name, website, state, weekend attendance, number of campuses or sites, and whether you operate a school or childcare program.
Insurance program details, such as whether you own or lease your buildings, approximate insured property value, renewal month, current annual premium, current broker and insurance company, and what prompted your request.
Uploaded documents, such as current insurance policies, declarations pages, schedules, and loss runs.
Messages you write to us in a form.
Checklist download requests. To download a checklist, we ask for details such as your name, email address, church name, weekly attendance, or number of campuses.
Intake information. If we move forward with a review or quote, we may send you a more detailed online intake form. It may ask for your organization's legal name, Federal EIN, addresses, staff and volunteer counts, payroll, budget, property details, programs, safety and screening practices, vehicles and drivers, events, and claims history. Some questions cover sensitive topics, such as whether there has been an allegation of abuse. Answer only what you are comfortable sharing, and tell us if you would rather discuss a question by phone.
Calls, texts, and emails. If you call, text, or email us, we receive the information you include, along with your phone number or email address.
Please do not send sensitive personal information about an individual, such as Social Security numbers, health information, or bank account numbers, through the Site. If a quote requires that kind of information, we will tell you how to send it securely.
Information collected automatically
When you visit the Site, we and our service providers automatically collect some technical information. This may include your IP address, browser and device type, the pages you view, the page that referred you, the date and time of your visit, and your general location (such as city or region) based on your IP address. We collect this using cookies and similar technologies, described in "Cookies and analytics" below.
How we use information
We use the information we collect to:
Respond to your questions and requests.
Review your current insurance program and prepare the quotes, benchmarks, and recommendations you ask for.
Send you the checklist or other resources you request.
Follow up with you about your request.
Place and service any insurance you buy through us.
Allow Provenance Risk to create de-identified data, as described in "How we share information."
Operate, secure, and improve the Site, including preventing spam and abuse of our forms.
Understand, in aggregate, how visitors use the Site.
Meet our legal, regulatory, and licensing obligations, and protect our rights.
We contact you using the details you give us. That may be an email, a phone call, or a text message if you gave us your phone number. If you download a checklist, we may send you a personal email or two about it. If you would rather not hear from us, tell us and we will stop.
We never sell information that identifies you or your church. We do not add you to a newsletter or marketing email list, and we do not send marketing text messages.
How we share information
We do not sell information that identifies you or your church, and we do not share it with anyone for their own marketing. We share information only as described below.
Insurance companies and markets. To get you a quote or place coverage you ask for, we share the information and documents needed with insurance companies, wholesale brokers, managing general agents, and similar insurance markets. We share only what is reasonably needed for your request. These companies handle information under their own privacy policies.
Our partner agency. We place some coverage through SmartChoice, our partner agency, which gives us access to additional insurance companies and licensing in other states. When we do, we share the information SmartChoice needs to quote, place, and service that coverage.
Companies that support your coverage. When your coverage calls for it, we may share information with premium finance companies, property inspection and valuation firms, and loss-control providers.
Service providers. We use companies that help us run the Site and our business. They process information for us, and we allow them to use it only to provide services to us. They include:
Squarespace, which hosts the Site and processes form submissions and uploaded files.
Cloudflare, which provides network delivery, security, and visitor analytics for the Site.
Google, which provides our business email and file storage (Google Workspace), Google Analytics, and reCAPTCHA spam protection on our forms.
Jotform, which hosts online intake forms we may send you.
Other software we use to store, organize, and analyze information, including spreadsheet, workflow, and artificial intelligence (AI) tools.
Provenance Risk. We use software from Provenance Risk, Inc. to organize intake information, analyze policies, and prepare insurance applications. Provenance Risk is a separate company. Our founder, Jake Lubinski, also founded Provenance Risk and has an ownership interest in it. Information you send us, including form submissions and uploaded documents, is loaded into Provenance Risk's software.
Provenance Risk uses identifiable information only to provide services to us and our clients. Provenance Risk never sells personal information or information that identifies your church. It may combine general characteristics, such as approximate location and church size, with information from other organizations to build de-identified datasets that help insurance companies underwrite more accurately. It may sell or license these datasets, including to insurance companies.
Provenance Risk keeps de-identified information in a form that cannot reasonably be linked to you, your church, or any person. It does not try to re-identify it, and it requires anyone who receives it to make the same commitments.
If Provenance Risk is sold to or merges with another company, including an insurance company, the information it holds may transfer to that company. Any identifiable information that transfers must stay protected under the limits described in this policy.
How we use AI tools. We use software, including AI tools, to help us read documents, organize information, and prepare summaries and applications. A licensed broker reviews the results before we rely on them or send them to an insurance company.
Legal and safety reasons. We may disclose information if we believe it is required by law, regulation, subpoena, or court order, or if insurance regulators request it. We may also disclose it to protect the rights, property, or safety of Hale Street Insurance, our clients, or others.
Professional advisers. We may share information with our lawyers, accountants, auditors, and insurers when needed for their services to us.
Business transfers. If we are involved in a merger, acquisition, sale of assets, or transfer of our book of business, information may be transferred as part of that transaction, subject to applicable law.
With your consent. We may share information for other purposes if you ask us to or agree to it.
Uploaded documents
When you request a program review, you can upload up to five documents, such as insurance policies, declarations pages, schedules, and loss runs. These documents can contain confidential business information about your organization.
Uploaded files reach us through Squarespace's form system. We then store them in our business email, cloud storage, and insurance software. We use them to review your program, prepare quotes and recommendations, and respond to your request. They are also loaded into Provenance Risk's software, as described in "How we share information." We share them with insurance companies and markets only as needed for the quote or coverage you ask for.
Only people who need your documents to help you can access them. We keep uploaded documents and form submissions as long as needed for your request, for any client relationship that follows, and to meet our legal, regulatory, and record-keeping obligations. After that, we securely delete them.
You can ask us to delete documents you uploaded. See "Your choices and requests" below. We may need to keep some records when the law requires it.
Phone calls and text messages
You can call or text us at 978.712.0111. If you text us, or give us your phone number in a form, we may reply by phone or text about your request. We do not send marketing text messages. Your mobile carrier's message and data rates may apply. Reply STOP, or tell us, and we will stop texting you.
We do not sell or share mobile phone numbers, or any consent to receive text messages, with third parties or affiliates for their marketing or promotional purposes.
Cookies and analytics
Cookies are small files stored on your device. We and our service providers use cookies and similar technologies to run the Site, keep it secure, and understand how it is used.
Essential cookies. Squarespace and Cloudflare use cookies needed for the Site to work and stay secure, such as protecting forms and detecting malicious traffic.
Analytics. We use Google Analytics to see, in aggregate, how visitors find and use the Site. We also use Squarespace's built-in visitor analytics and Cloudflare Web Analytics. These tools may collect information such as pages viewed, time on the Site, referring sites, device type, and approximate location. Because Google Analytics uses cookies, Google may collect information about your use of the Site over time and across other websites that use its services. We have not turned on Google signals or personalized advertising in Google Analytics, and we do not use Site data to build advertising audiences.
Spam protection. Our forms use Google reCAPTCHA. It collects hardware and software information, such as device and browser data, and sends it to Google to check that a person, not a bot, is submitting the form. Google's Privacy Policy and Terms of Service apply to reCAPTCHA.
Intake forms. Online intake forms we send you are hosted by Jotform on its own website. Jotform may use its own cookies, as described in its privacy policy.
We do not use advertising or retargeting cookies or pixels on the Site.
Your options.
Most browsers let you block or delete cookies. Blocking some cookies may affect how the Site works.
You can opt out of Google Analytics with the Google Analytics Opt-out Browser Add-on.
You can learn more at How Google uses information from sites or apps that use our services.
Do Not Track and Global Privacy Control. Some browsers send a "Do Not Track" signal. There is no common standard for responding to it, so the Site does not respond to Do Not Track signals. Because we do not sell personal information that identifies you or use it for targeted advertising, a Global Privacy Control signal does not change how we handle your information.
How we protect information
We use reasonable administrative, technical, and physical safeguards to protect the information we collect. The Site uses an encrypted (HTTPS) connection, and Squarespace and Cloudflare provide hosting and security protections. We limit access to submitted information to people who need it to help you.
No website, email, or storage system is completely secure, so we cannot guarantee absolute security. If you are concerned about sending a document through the Site, contact us and we will arrange another way.
Children's privacy
The Site is meant for adults who act for churches and other organizations. It is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. If you believe a child has sent us personal information, contact us at [email protected] and we will delete it.
Your choices and requests
You can ask us to:
tell you what personal information we have about you,
correct information that is inaccurate,
delete information you sent us through the Site,
keep your information out of Provenance Risk's de-identified datasets, or
stop contacting you.
To make a request, email [email protected]. We may need to verify your identity before we act on it. We will respond within 45 days. Sometimes we may need to keep information, for example to meet legal, regulatory, or record-keeping requirements, or for an insurance policy that is in force. If so, we will tell you.
You can also choose not to submit information through the Site. If you do not, we may not be able to provide a review or quote.
State privacy rights
Depending on where you live, state privacy laws may give you additional rights. These may include the right to know what personal information is collected and how it is used and disclosed, to request correction or deletion, and to not be discriminated against for using your rights. Whether a particular law applies to us depends on factors such as our size, the amount and type of information involved, and exemptions for information covered by insurance and financial privacy laws.
If you would like to make a request under a state privacy law, contact us at [email protected]. We will respond as required by the laws that apply to us.
Insurance privacy notices
As an insurance producer, we may have separate legal duties for "nonpublic personal information" about individual consumers and customers under the federal Gramm-Leach-Bliley Act and state insurance privacy laws. If those duties apply to our relationship with you, we will provide any required privacy notice separately, and that notice will control for the information it covers. Insurance companies that quote or issue your coverage may also send you their own privacy notices.
Links to other websites
The Site links to other websites, such as insurance company and resource sites, and we may send you links to intake forms hosted by Jotform. Those sites are governed by their own privacy policies, and we are not responsible for how they handle information. Please review their policies before you submit information.
Changes to this policy
We may update this Privacy Policy from time to time. When we do, we will post the new version on this page and update the effective date above. If we make a significant change in how we use or share information you have already given us, we will take reasonable steps to let you know, as required by law.
